Job Summary
We are seeking an experienced Patient Advocate Specialist to join our team. As a key member of the case management team, you will be responsible for advocating on behalf of patients and ensuring their needs are met through effective care coordination.

About the Role
The successful candidate will have a strong background in healthcare, preferably with experience in case management or a related field. You will work closely with patients, employers, and other stakeholders to develop and implement tailored advocacy plans that promote positive outcomes.

Responsibilities
- Advocate on behalf of patients to ensure their needs are met through effective care coordination
- Develop and implement individualized advocacy plans to achieve optimal patient outcomes
- Collaborate with patients, employers, and other healthcare professionals to ensure seamless care coordination
- Conduct regular assessments and reviews to monitor progress and adjust advocacy plans as needed
- Maintain accurate records and reports of patient interactions and advocacy plans

Requirements
- Bachelor's degree in a relevant field (e.g., occupational therapy, physiotherapy, nursing)
- Proven experience in case management or a related healthcare field
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Familiarity with case management software and systems
