About the Role

We are seeking a highly skilled and experienced Nurse Therapist to join our Lymphoedema team at South Bucks Hospice. As a key member of our team, you will be responsible for providing high-quality care and treatment to patients suffering from Lymphoedema.

Key Responsibilities
  • Provide treatment and care to patients with Lymphoedema, ensuring their comfort and long-term health care needs are met.
  • Work collaboratively with the team to assess, plan, implement, and evaluate patient care, coordinating care from admission to discharge.
  • Evaluate referrals from medical and non-medical practitioners, conducting initial assessments for patients referred for Lymphoedema treatment.
  • Support community teams in their practice of patients with Lymphoedema, taking a wider view of patient care.
  • Refer patients to other professionals as necessary, maintaining patient safety and providing physical, emotional, and psychological support.
  • Ensure 100% compliance with professional, ethical patient assessments, care plans, and note-taking, maintaining clear and accurate records.
  • Deliver education on the management of Lymphoedema, as agreed with the Head of Lymphoedema.
  • Support the department and activities to ensure compliance with NICE and CQC guidelines.
  • Manage patient caseloads in accordance with South Bucks Hospice's policies and procedures.
  • Deputise for the Head of Lymphoedema in their absence.
  • Support service development and achievement of departmental aims and objectives.
  • Participate in clinical and hospice initiatives to achieve the strategic aims of South Bucks Hospice.
  • Recognize and take positive action to support the team, working towards an holistic approach to staff wellbeing.
  • Comply with staff appraisals, performance reviews, training, and education.
  • Accurately report on the performance, operational needs, and general capacity to the Head of Lymphoedema.
  • Actively comply with and demonstrate the implementation of good infection prevention control in accordance with policy and procedures.
  • Support the writing of policies and procedures for South Bucks Hospice to achieve best practice.
Person Specification
  • Appropriate Nursing Qualification and minimum of two years' experience within a similar position.
  • Good understanding of Lymphoedema.
  • Demonstrate a consistent high standard of work and attention to detail.
  • Strong numerical and verbal skills.
  • Advance Excel and Word skills.
  • Punctual and presentable.
  • Approachable and accessible.
  • Willing to learn.
  • Communicate with empathy and understanding towards patients and staff.
  • Ability to listen and act appropriately.
  • Effective communication with colleagues and Management team.
  • Calm temperament under pressured conditions.
  • Willingness to travel to patient locations.
