Job Opportunity: Band 6 Mental Health Practitioner

We are seeking an experienced and motivated Band 6 Mental Health Practitioner to join our multidisciplinary team in Milton Keynes. As a key member of our team, you will be responsible for conducting comprehensive biopsychosocial assessments for patients presenting in the Emergency Department and general wards at Milton Keynes University Hospital with mental health crisis and/or mental health problems.

Key Responsibilities:

  • Conduct comprehensive biopsychosocial assessments for patients in crisis
  • Develop and implement treatment plans in collaboration with multidisciplinary teams
  • Facilitate discharge planning and pathway planning for patients
  • Collaborate with community health services to provide seamless care
  • Participate in teaching and training sessions for staff

Requirements:

  • Substantive post-registration experience as an RMN, Occupational Therapist, or Social Worker
  • Relevant knowledge and experience in discharge planning and Adult Social Care
  • Excellent risk assessment and decision-making skills
