Support Coordinator Job Description

We are seeking a dedicated and compassionate Support Coordinator to join our team at Creative Support. As a key member of our Floating Support Service in Gateshead, you will play a vital role in providing outcome-focused care and support to individuals at risk of homelessness and in need of housing-related provision.

Key Responsibilities:

  • Support individuals with complex needs and requirements, helping them access facilities to find and maintain tenancies in the local community.
  • Work collaboratively with our team to provide a comprehensive support package, addressing the unique needs of each individual.
  • Develop and maintain strong relationships with local community groups, services, and stakeholders to ensure seamless support for our clients.
  • Contribute to the development and implementation of individual support plans, ensuring that each client receives tailored support to achieve their goals.

Requirements:

  • Experience working with vulnerable adults, particularly those at risk of homelessness.
  • Strong communication and interpersonal skills, with the ability to build rapport with clients, colleagues, and external partners.
  • Ability to work independently and as part of a team, with a flexible approach to meet the changing needs of our clients.
  • Knowledge of housing-related provision and support services, with a commitment to delivering high-quality, person-centered care.
