Support Services Coordinator

We are seeking a highly motivated and compassionate Support Coordinator to join our team at Creative Support. As a Support Coordinator, you will play a vital role in providing outcome-focused care and support to individuals who are at risk of homelessness and in need of housing-related provision.
Key Responsibilities:- Support individuals with complex needs and requirements, helping them access facilities to find and maintain tenancies in the local community.
- Work collaboratively with our team to provide a comprehensive support service that addresses the unique needs of each individual.
- Develop and maintain strong relationships with local community groups and organizations to ensure our service users receive the best possible support.
- Contribute to the development and implementation of our service's policies and procedures.
