Support Coordinator Job Description

We are seeking a highly motivated and compassionate Support Coordinator to join our team at Creative Support. As a Support Coordinator, you will play a vital role in providing outcome-focused care and support to individuals who are at risk of homelessness and in need of housing-related provision.

Key Responsibilities:
  • Support individuals with complex needs and requirements, helping them access facilities to find and maintain tenancies in the local community.
  • Work collaboratively with our team to provide a comprehensive support service that addresses the unique needs of each individual.
  • Develop and maintain strong relationships with local community groups and organizations to ensure access to resources and services.
  • Contribute to the development and implementation of individual support plans, ensuring that each person's needs are met and their goals are achieved.
