Support Coordinator Job Description

We are seeking a highly motivated and compassionate Support Coordinator to join our team at Creative Support. As a Support Coordinator, you will play a vital role in providing outcome-focused care and support to individuals who are at risk of homelessness and in need of housing-related provision.

Key Responsibilities:

  • Support individuals with complex needs and requirements, helping them access facilities to find and maintain tenancies in the local community.
  • Work collaboratively with our team to provide a comprehensive support service that meets the unique needs of each individual.
  • Develop and maintain strong relationships with service users, their families, and other stakeholders to ensure a seamless support experience.
  • Contribute to the development and implementation of individual support plans, ensuring that each person receives the support they need to achieve their goals.

Requirements:

  • Experience working in a support role, preferably in a housing-related provision or homelessness service.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with service users and stakeholders.
  • Ability to work collaboratively as part of a team, with a flexible and adaptable approach to meet the changing needs of the service.
  • Knowledge of housing-related provision and homelessness services, with a commitment to delivering high-quality support that meets the needs of each individual.
