Support Services Coordinator

We are recruiting a Support Coordinator for our Floating Support Service in Gateshead. Our service provides outcome-focused care and support to individuals who are at risk of homelessness and in need of housing-related provision.
About the Role:You will be an active part in supporting individuals with complex needs and requirements, helping them access facilities to find and maintain tenancies in the local community. At Creative Support, we are committed to providing high-quality care and support that enables individuals to live fulfilling lives.
Key Responsibilities:- Support individuals with complex needs and requirements
- Help individuals access facilities to find and maintain tenancies
- Work collaboratively with other agencies to provide a comprehensive support package
- Monitor and review individual plans to ensure effective support
To be successful in this role, you will need to have a strong understanding of the needs of individuals at risk of homelessness and a commitment to providing high-quality care and support. You will also need to be able to work collaboratively with other agencies and individuals to provide a comprehensive support package.
