About the Role

Job Description

The successful candidate will be responsible for coordinating and monitoring complex care packages, conducting thorough assessments, and working closely with healthcare professionals and providers. This role requires strong clinical expertise, excellent communication skills, and the ability to work autonomously.

As an Assessment and Care Coordinator, you will play a crucial role in improving the lives of individuals through high-quality care assessments and management. You will work within a supportive team that values collaboration and excellence in care, with opportunities for continuous learning and professional growth.

In this role, you will be required to:

  • Assess patients' needs and develop individualised care plans.
  • Coordinate and monitor care packages to ensure effective delivery of services.
  • Work closely with healthcare professionals and providers to achieve positive outcomes.

Requirements

To be successful in this role, you will need:

  • A relevant degree or professional qualification in Nursing (local areas may differ/could be Social Work).
  • Evidence of continuous professional development.
  • Extensive experience of case management with service users who have complex health needs.
  • A clear knowledge and understanding of The National Framework for NHS Continuing Health Care and its application in practice.

Salary and Benefits

The estimated salary for this role is £40,000 - £55,000 per annum, depending on experience. We offer a range of benefits, including:
