Practice Nurse

Pickhurst Surgery, a leading healthcare provider in Bromley, Hayes, is seeking an experienced Practice Nurse to join our dynamic team. As a key member of our extended clinical team, you will play a vital role in delivering high-quality patient care and contributing to the success of our Primary Care Network.
Key Responsibilities:- Implement evidence-based practice for the effective management of patients with long-term conditions, including care for elderly and housebound patients.
- Work as a senior member of the practice multidisciplinary team to ensure nursing services are delivered effectively to the entitled patient population.
- Develop and implement health promotion and well-being programs for the wider population, in collaboration with the practice management team.
- Participate in multidisciplinary clinics, patient reviews, and team meetings, ensuring accurate coding and recording of MDT meetings in Emis.
- Contribute to the practice achieving its PMS contract obligations, including quality, enhanced services, Public Health, organizational, and access targets.
- Ensure all data protection requirements are met when gathering, recording, and storing patient data.
- Support the Infection Prevention Control (IPC) lead and work to the IPC policy, implementing audits, training, support, and evidence of compliance.
- Participate in the review of significant and near-miss events, applying a structured approach, such as root cause analysis (RCA).
- Support the practice team with reviewing and implementing practice policies and protocols, ensuring conformance to extant legislation.
- Nursing degree or equivalent experience.
- Registered Nurse (NMC).
- Evidence of immunization training and assessment of competence in the last 2 years.
- NMC's 'Academic Assessors', 'Practice Supervisors', and 'Practice Assessors' accreditations.
- Postgraduate diploma or degree (Primary Care).
- Immunizations (flu, routine, childhood, B12, travel).
- Wound Care / Treatment of small injuries / Removal of sutures & staples.
- Women's health (Cervical cytology, contraception, etc.).
- Quality of Framework (QOF) understanding.
- Requesting pathology tests and processing the results, advising patients accordingly.
- Understanding the requirement for PGDs and associated policy.
- 6 months' relevant experience working in a primary care environment in the last 2 years.
- Travel vaccination and medicine.
- Clear, polite telephone manner.
- Understanding the need for confidentiality & data awareness issues.
- Leadership and management skills, as part of general practice nursing team with delegated accountability and responsibility for patient care.
- NHS primary care coding.
- Competent in the use of Office and Outlook.
- Effective time management (Planning & Organising).
- Ability to follow policy and procedure.
- Experience with clinical risk management.
- Flexible and cooperative.
- Commitment to ongoing professional development.
- Flexibility to work outside of core office hours.
- Disclosure Barring Service (DBS) check.
