Job Title: Senior Occupational Therapist

We are seeking an experienced and motivated Senior Occupational Therapist to join our innovative and dynamic community service. As a key member of our team, you will be responsible for providing high-quality patient-centered care and contributing to the development of our service.

Main Responsibilities:
  • Provide occupational therapy services to patients in their own homes, focusing on maintaining independence and preventing hospital admission.
  • Work closely with a multidisciplinary team, including physiotherapists, nurses, and healthcare assistants, to ensure seamless care delivery.
  • Develop and implement individualized care plans, taking into account patients' needs, goals, and preferences.
  • Conduct assessments, interventions, and evaluations to ensure optimal patient outcomes.
  • Collaborate with other healthcare professionals to ensure comprehensive care and address any concerns or issues that may arise.
Requirements:
  • Experience working at B5/B6 level in occupational therapy.
  • Previous NHS experience, preferably in community rehabilitation.
  • HCPC registration and a degree in occupational therapy.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Knowledge of occupational therapy processes and principles, with a focus on patient-centered care.
