Complex Care Manager Job Description

We are seeking an experienced and passionate Complex Care Manager to join our team at Leaders In Care Recruitment Ltd. As a Complex Care Manager, you will be responsible for delivering high standards of care and making a real difference to the lives of our clients.

Key Responsibilities:
  • Conduct thorough assessments and implement personalized care plans to meet the unique needs of our clients.
  • Induct and supervise support workers to ensure they have the skills and knowledge to provide exceptional care.
  • Perform spot checks and risk management to ensure the safety and well-being of our clients.
  • Attend meetings with Integrated Care Boards (ICBs) and clients to discuss care plans and progress.
  • Provide regular reports and updates to stakeholders.
Requirements:
  • NVQ Level 3 (or equivalent) in Health and Social Care or a related topic.
  • Proven track record in managing and coordinating care for clients in a community setting.
  • Experience in developing and implementing care plans.
  • Supportive management style that can bring out the best in others.
  • Advanced communication skills verbally and written.
  • Full UK driving licence for client visits.
