About the Role

We are seeking a highly skilled and experienced Mental Health Practitioner to join our Older People's Community Mental Health team. As a key member of our team, you will be responsible for providing high-quality care and support to individuals with complex mental health needs.

Key Responsibilities

  • Assess and manage risk for patients with organic and functional presentations under CPA and NCPA
  • Use Dialog+ and Recovery Care Planning with service users and carers
  • Manage your own diary and plan your day effectively
  • Attend MDT meetings and wards
  • Participate in the Duty Rota
  • Communicate effectively through notes on Lorenzo and letters to Service Users, GPs and other involved parties
  • Administer depot medication
  • Drive independently for this role
