About the Role

We are seeking an experienced Mental Health Practitioner to join our dynamic and supportive Older People's Community Mental Health team.

As a key member of our team, you will work closely with a close-knit multi-disciplinary team of professionals, including Assistant Practitioners, Nurses, Occupational Therapists, Psychological Therapists, Independent Prescribers, and Psychiatrists.

You will be responsible for managing a caseload of patients with organic and functional presentations under CPA and NCPA, assessing and managing risk, using Dialog+ where possible, and Recovery Care Planning with service users and carers.

The successful candidate will have a strong interest in working in Older People's services, physical as well as mental health, and understanding of assessment, referral process, case handling, and working collaboratively with the wider MDT.

Key Responsibilities
  • Holding a caseload of patients with organic and functional presentations under CPA and NCPA
  • Assessing and managing risk
  • Using Dialog+ where possible and Recovery Care Planning with service users and carers
  • Managing your own diary and planning your day
  • Attending MDT meetings and wards
  • Participating in the Duty Rota
  • Communicate effectively through notes on Lorenzo and letters to Service Users, GPs, and other involved parties
  • Administering depot medication
