About This Role

We are seeking a skilled and compassionate Community Mental Health Practitioner to join our dedicated team at Norfolk & Suffolk Foundation NHS Trust.

As a key member of our Older People's Mental Health service, you will work closely with a multi-disciplinary team to provide high-quality care and support to individuals with complex needs.

Key Responsibilities:

  • Manage a caseload of patients with organic and functional presentations under CPA and NCPA
  • Assess and manage risk
  • Use Dialog+ where possible and Recovery Care Planning with service users and carers
  • Communicate effectively through notes on Lorenzo and letters to Service Users, GPs and other involved parties
  • Administer depot medication
