About the Role

We are seeking a highly skilled and experienced Mental Health Practitioner to join our team at Penwith Primary Care Network. As a key member of our multidisciplinary team, you will be responsible for providing holistic and person-centred primary care mental health services to our patients.

Key Responsibilities
  • Provide comprehensive assessments and develop individualized care plans for patients with mental health needs.
  • Deliver evidence-based interventions and therapies to support patients in achieving their mental health goals.
  • Work collaboratively with patients, families, and other healthcare professionals to ensure seamless care coordination.
  • Participate in clinical audits and research projects to inform best practices and improve patient outcomes.
  • Supervise and mentor junior staff members to ensure high-quality care and professional development.
Requirements
  • Professional registration with the NMC or equivalent registering professional body.
  • Postgraduate qualification in a relevant mental health discipline (e.g., psychology, social work, occupational therapy).
  • Minimum 2 years of experience working in mental health services, with a focus on primary care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse patient populations.
  • Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities.
