This specialized role focuses on working closely with women across the county, addressing substance use, healthy relationships, mental, and emotional health through a trauma-informed outreach model. You will work collaboratively with various agencies to develop sustainable pathways and support women in meeting their objectives.

Your primary responsibility will be to help clients identify their ambitions, passions, and goals, creating personalized treatment plans tailored to their needs. As a frontline worker, you will deliver continuity of care through effective partnership working, care coordination, intervention delivery, and key work responsibilities. Your caseload will consist of active clients, requiring you to establish meaningful connections and treat them with respect and dignity.

To excel in this role, you will need to possess excellent communication skills, be highly organized, and able to manage your time effectively. We are seeking a compassionate and motivated individual with demonstrable experience working within healthcare, social care, criminal justice, or other specialist settings, preferably with those experiencing substance misuse issues. A positive and engaging approach towards service users and stakeholders is essential.
