Mental Health Practitioner

About the Role
We are seeking a highly skilled and experienced Mental Health Practitioner to join our team at Penwith Primary Care Network. As a key member of our multidisciplinary team, you will be responsible for providing holistic and person-centred primary care mental health services to our patients.
Key Responsibilities
- Provide comprehensive assessments and develop complex care plans for patients with mental health needs.
- Deliver evidence-based interventions and work closely with mental health and wellbeing coaches to formulate and deliver care pathways.
- Participate in daily huddle/safety meetings and oversee staff member workloads to ensure safe and effective service delivery.
- Work closely with NHS colleagues to review serious incident investigations and identify key areas for quality improvement.
Person Specification
- Professional qualification in mental health nursing, social work, or occupational therapy.
- Experience of working with people with mental illness and those at risk of becoming unwell.
- Ability to work autonomously and make decisions in a fast-paced environment.
- Excellent communication and interpersonal skills.
