Community Outreach Assistant

We are seeking a part-time support practitioner to join our team in the Motherwell and Wishaw areas. The successful candidate will have a full UK manual driving licence and access to our mobility cars, which will enable them to transport service users to appointments and activities.
ResponsibilitiesThe key responsibilities of this role include:
- Providing person-centered support to individuals in their homes
- Assisting with daily living tasks such as meal planning, cooking, and housework
- Transporting service users to appointments and activities using our mobility cars
- Working collaboratively with colleagues to achieve high-quality outcomes
