Mental Health Transition Coordinator

We are seeking a highly skilled Mental Health Transition Coordinator to join our team in supporting the transition of young adults aged 18-25 into Community Mental Health Teams across the North West. As a key member of our team, you will engage with young adults, their families, and referrers to provide informed decision-making around treatment plans and transition.
The successful candidate will strive to achieve NICE transition standards and promote Mersey Care principles of transition, ensuring seamless care coordination between services. You will develop strong relationships with stakeholders across the system, working closely with Children and Young People services to support transitions throughout primary care and VCFSEs.

Key Responsibilities:
- Develop and implement effective transition plans for young adults aged 18-25
- Coordinate care and support before, during, and after transfer
- Facilitate meetings with practitioners from each adult service the young person will transition to
- Engage with young people who do not attend initial meetings or appointments to provide further opportunities to engage
This role requires a high degree of initiative, flexibility, and problem-solving skills, with the ability to work independently and as part of a multidisciplinary team. You will be working within the CMHT, supporting young adults for a limited time period until they have successfully achieved their transition goals and begun treatment within the team.
The ideal candidate will possess excellent communication and interpersonal skills, with the ability to educate and support other professionals. You will also have experience in providing care for individuals with mental health difficulties within a secondary care environment, including risk formulation and management.
