Mental Health Care Coordinator

We are seeking an experienced Adult Mental Health Practitioner to join our team in providing exceptional care to patients with mental health issues.
Job SummaryThis role involves working as part of a multidisciplinary team to deliver high-quality clinical care to adults with complex mental health needs.
Key Responsibilities- Assess and develop care plans for patients with complex mental health conditions, collaborating with other healthcare professionals to ensure effective treatment and support.
- Maintain accurate records of patient interactions, including progress notes, medication administration, and any concerns or issues that arise during care provision.
- Work collaboratively with colleagues to promote a culture of mutual respect, open communication, and continuous learning, adhering to best practices in mental health care.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that enhance patient outcomes and staff satisfaction.
- Support the development and implementation of new services and programs aimed at improving access to mental health services and promoting community integration.
- A relevant professional qualification (RMN, AHP, Social Worker) and current registration with the relevant regulatory body.
- Proven experience working with adult populations, preferably in a mental health setting, with a focus on delivering individualized care and support.
- A strong understanding of mental health legislation, policy, and guidelines, particularly the Mental Health Act 1983 and associated regulations.
- Demonstrated skills in assessment, diagnosis, and treatment planning, as well as experience with crisis intervention and management techniques.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, carers, and other healthcare professionals.
The estimated annual salary for this position is approximately £35,000 - £40,000, depending on qualifications and experience.
