We are currently seeking an experienced Emergency Response Care Specialist to join our dynamic team in Norfolk. As a key member of our crisis response service, you will be responsible for providing high-quality care support to customers and their families at short notice.


Job Summary


This exciting role offers the opportunity to work in a fast-paced environment, where no two days are the same. You will be part of a responsive team, providing emergency care support in the community, including hospital avoidance and breaks for carers.


As an Emergency Response Care Specialist, you will be responsible for:



  • Providing person-centred care support in the community, including complex cases
  • Traveling as required to deliver support
  • Signposting patients and their families to relevant support services to prevent further admissions
  • Working closely with the bookings team to identify support needs
  • Completing risk assessments and care plans for new and existing customers

Requirements


To be successful in this role, you will need:



  • A minimum NVQ Level 2 in health and social care or equivalent
  • Experience of providing person-centred care in the community
  • Good IT skills, including experience using laptops and rostering systems
  • Ability to work independently, use your own initiative, and problem-solve
  • Excellent communication and interpersonal skills
