About the Role


This is an exciting opportunity for a skilled and enthusiastic Community Nurse to join our team at Holywell Community Hospital. The successful candidate will be responsible for providing high-quality nursing care to patients in a community setting, working collaboratively with other healthcare professionals to deliver effective patient outcomes.


The post holder will be involved in clinic work and community visits, requiring strong communication and interpersonal skills. This fixed-term / secondment position is available due to maternity cover and is expected to last months.


Main Responsibilities


The successful candidate will be responsible for assessing, planning, implementing, and evaluating evidenced-based nursing care, working closely with patients and their families to meet their needs. They will also assist in the management and organisation of their clinical area, participating in education, development, and supervision of other staff members.


Additionally, the ability to speak Welsh is desirable for this role, although English and/or Welsh speakers are equally welcome to apply.
