Main Responsibilities- Deliver high-quality care and interventions to patients by demonstrating up-to-date evidence-based practice.
- Ensure medication is administered in accordance with risk assessment and policy.
- Develop programmes of care for patients that meet their needs for chronic disease and acute episodes of care.
- Collaborate with multi-disciplinary teams to support seamless care provision for those identified with complex health needs.
- Liaise with mental health and substance misuse services for allocated patients.
This role offers the chance to empower patients to take responsibility for their health, wellbeing, recovery, and future lifestyle. As a key member of our team, you will maintain accurate clinical records and care plans in accordance with NMC standards.
About the RoleThe successful candidate will have experience working within secure environments and possess a strong understanding of local and national key documents impacting healthcare delivery in prisons. You will be able to work effectively within a multi-disciplinary team, using your knowledge of current healthcare issues to provide high-quality patient care.
What We Offer- A competitive salary of £35,000 - £45,000 per annum, dependent on experience.
