Care Coordinator

We are looking for a highly skilled Care Coordinator to join our Access Team. As a key member of the team, you will be responsible for coordinating assessment clinics and prioritising appointments for individuals referred to the service.
Main Responsibilities:
- Coordinate assessment clinics and prioritise appointments.
- Undertake comprehensive risk assessments and develop individualised packages of care.
- Provide education and appropriate self-help resources to patients specific to individual needs.
Requirements:
- A non-medical prescribing qualification.
- A formal teaching qualification.
- Further training in brief psychosocial interventions.
- Experience of working in multi-disciplinary teams.
Salary: £38,000 - £42,000 per annum.
