About the Role:

The Care and Support Coordinator will be part of a team providing responsive, effective, and flexible housing management and support to customers in Independent Living. This role is responsible for promoting and maximising independence for older people within a user-led environment, aligning with the Great Places Service Commitment framework.

Duties and Responsibilities:
  • Support customers, involving relevant parties where necessary, to maintain their independence and thrive in their tenancy and community.
  • Manage all aspects of tenancy, including voids, allocations, managing tenancy ends and new tenancies, customer inductions, rents, ASB and challenging behaviour, and breaches of occupancy agreements, as well as monitoring repairs, maintenance, and adaptations.
  • Oversee general building management, health and safety, security, and building compliance.
  • Support customer welfare, conducting welfare checks according to the service offer, responding to alarm calls and emergencies when on site, and collaborating with customers' relatives and support agencies to ensure their housing and support needs are met.
  • Create Personal Safety Plans with all customers as required, taking a person-centred approach to advise and guide customers to maintain their security and safety, helping them identify risks and how these can be managed.
  • Record accurate and timely information as needed for the role and in line with Data Protection regulations, ensuring that data is recorded and shared as required to support customers and maintain service safety.
  • Promote partnership working with other agencies, signposting their services to customers where appropriate, and supporting access to relevant services.
  • Encourage customers to participate in social activities, engaging with other agencies and the wider community.
  • Address safeguarding concerns promptly and take appropriate action.
Requirements:
  • Experience in safeguarding vulnerable adults.
  • Understanding of administration and recording procedures, with knowledge of confidentiality.
  • Working experience applying Health & safety legislation.
  • Relevant professional qualification or membership, or equivalent experience.
  • Experience in basic housing processes and welfare benefits.
  • Knowledge of current housing and tenancy legislation and procedures to inform decisions.
  • Able to travel to multiple locations.
  • Driving licence and access to transport.
Estimated Salary:

£26,313 per annum
