District Nurse Specialist

About the Role
We are seeking a highly skilled and experienced District Nurse to join our team in Manchester. As a District Nurse Specialist, you will be responsible for providing high-quality nursing care to patients in their own homes and in various community settings.
Key Responsibilities
- Lead and coordinate nursing teams to deliver complex nursing care to adults in the community
- Work closely with multidisciplinary teams to ensure effective patient care and coordination
- Develop and implement nursing plans to meet the needs of patients with complex health needs
- Provide leadership and guidance to junior members of staff
- Contribute to the development of nursing services and policies
Requirements
- Registered Nurse with a Community Specialist Practitioner qualification
- Experience of working in community nursing and leading teams
- Strong communication and interpersonal skills
- Ability to work autonomously and as part of a team
- Computer literate and able to use electronic records systems
