About the Role
We are seeking a highly skilled and experienced District Nurse to join our Community Nursing Team as a Community Specialist Practitioner. As a key member of our team, you will be responsible for providing high-quality nursing care to patients in their own homes and community settings.
Key Responsibilities
- Lead and co-ordinate nursing teams to provide complex nursing care to adults in their own homes and community settings.
- Work closely with the multidisciplinary team (MDT) to ensure effective patient care and coordination.
- Develop and implement care plans that meet the individual needs of patients.
- Provide education and support to patients, families, and carers to promote health and wellbeing.
- Contribute to the development and implementation of policies and procedures to improve patient care and outcomes.
Requirements
- Registered Nurse with a Community Specialist Practitioner qualification.
- Experience of working in a community nursing setting.
- Ability to work autonomously and as part of a team.
- Excellent communication and interpersonal skills.
- Ability to travel throughout different locations.
