Community Learning Disability Nurse Specialist

The post is a 12-month fixed-term position, created to cover a permanent staff member on secondment. As a member of the multidisciplinary health team in Brent, you will provide high standards of clinical assessment and intervention relevant to your specialist area.
Main Duties- Provide appropriate support and contact for service users, taking into account their health, social, and cultural needs, through thorough assessment and planning of the client's health needs.
- Ensure that clients are consulted about their care at all times and are involved in decision-making concerning their care.
- Carry out risk assessments for clients in line with the Trust and other relevant policies and procedures.
- Develop and maintain innovative and creative ways of working with clients who are difficult to engage.
- Provide clinical supervision to junior staff and students.
- Ensure that accurate clinical notes are kept and audited to ensure compliance with records-keeping standards of the Trust and LD Service.
- Take responsibility for ensuring that clinical practices are in keeping with relevant legislations and policies, and follow best practice guidelines.
- Be professionally accountable for own practice to your head of profession. Ensure that service users and their families are fully involved in the provision of care and services.

Job ResponsibilitiesTo work as an integral member of the learning disabilities team, enabling people to access mainstream health services, manage a caseload of clients with learning disabilities who have a range of health needs, using evidence-based and person-centred principles to plan, implement, and evaluate interventions within the CLDT.

Person SpecificationQualifications
Essential
- Current Professional Registration and qualification in Nursing RNLD.
- Evidence of post-registration training and continuing professional development in mandatory and statutory areas, such as safeguarding, Mental Capacity Act, including capacity assessments, Mental Health Act, Health and Safety, Manual Handling, Infection Control, Emergency First Aid, Confidentiality, and CPA or willingness to undertake training in these areas, relevant to your professional registration.
Desirable
- Evidence of post-registration training and continuing professional development in areas relevant to Learning Disabilities, such as challenging behaviour, autism, and mental health problems.
Previous Experience
Essential
- Previous experience of working with Learning Disabilities service users collaboratively to form care plans.
- Previous experience of working with people who have learning disabilities, serious and enduring mental health problems, challenging behaviour in a variety of different settings (community and inpatient) for people with learning disabilities.
- Post-registration experience of working in Learning Disabilities.
Desirable
- Supervision of students and junior staff.
- Previous experience of delivering teaching sessions to a variety of audiences.
- Previous experience of delivering evidenced successful care packages.
Skills, Knowledge, Abilities
Essential
- Possess extensive knowledge of CPA, with the ability to assess, implement, plan, and evaluate care.
- Possess extensive knowledge of the Mental Health Act 1983 and 2007 amendment, including supervised community treatment, The Mental Capacity Act 2005, including capacity assessments, and the attendant Codes of Practice.
- Demonstrate good understanding of issues around the delivery of learning disabilities services to a diverse multi-ethnic population within an inner city/urban geographical area.
- Ability to carry out risk assessments. Sound evidence of good clinical skills/knowledge base, particularly in the areas of risk assessment, care planning, care implementation, and co-ordination.
- Demonstrate excellent problem-solving skills and the ability to reflect, make safe clinical decisions, and act on own initiative.
- To have proven ability in the use of IT systems.
- Understanding of own professional code of ethics.
- Knowledge of safeguarding policies and procedures regarding both children and adults.
Desirable
- Knowledge of recent and current issues in learning disabilities and their impact on community mental health care.
- Evidence of the ability to conduct research/audit and the implementation of evidence-based clinical practice.
