Are you passionate about delivering exceptional support and care to individuals living with frailty and life-limiting conditions? We are seeking a Rehabilitation and Care Coordinator to join our award-winning Rehab and Wellbeing Service team at Garden House Hospice Care.

In this role, you will work closely with occupational therapists and physiotherapists to develop and implement tailored rehabilitation plans.

Responsibilities:

  1. Conduct thorough assessments to identify patient needs and goals.
  2. Collaborate with the multidisciplinary team to deliver high-quality care and support.
  3. Manage a designated caseload, ensuring timely interventions and goal attainment.

Requirements:

  • NVQ level 3 or equivalent qualification in a related field.
  • Current driving license and access to a car for business purposes.

Garden House Hospice Care Offers:

  • Continuation of NHS pension and recognition of long service.
  • 27 days annual leave + Bank Holidays + additional holiday after 5 and 10 years.
