Hospital Discharge Coordinators play a vital role in ensuring the smooth transition of patients from hospitals to their homes or other care settings.

About the Opportunity

We are seeking an experienced and dedicated individual to join our Hospital Discharge Team at Austin Fraser.

  • You will work closely with healthcare professionals, families, and other stakeholders to provide comprehensive support to vulnerable adults in a hospital setting.
  • Using your expertise, you will conduct thorough assessments, write detailed reports, and develop tailored care plans to ensure effective discharge processes.
  • You will manage complex cases, navigate challenging situations, and liaise with other professional bodies to ensure seamless coordination and communication.

Required Qualifications: A Qualified Social Worker, registered with Social Work England, holding a Social Work qualification (Diploma in social work, social work degree or equivalent) and a minimum of 3 years' experience post-qualifying as a Qualified Social Worker.

The estimated salary for this role is £35 per hour.
