About the Role

We are seeking an experienced Medicines Management Pharmacy Technician to join our team as a Medicines Optimisation Specialist. This is a unique opportunity to enhance our ability to provide medicines support for Acute Hospital at Home patients.

Job Summary

The successful candidate will be based in Barnstaple or Exeter and work to support patient care for AHAH patients across our Northern and Eastern services.

Main Responsibilities:
  • To provide a safe and cost-effective Medicines Management Pharmacy service to the AHAH service.
  • To be a source of medicines advice to colleagues, patients, and key stakeholders with respect to medicines optimisation for AHAH patients.
  • Facilitate safe transfer of patients to and from AHAH service (including discharge) with respect to medications by ensuring appropriate medicines are available and optimised.
  • Communicate effectively and promptly with stakeholders to ensure relevant information available to Health and Social Care staff, patients, and family members following patient transfer and discharge with respect to medicines.
Requirements:
  • A sound knowledge and experience of hospital pharmacy at ward level supporting medicines reconciliation.
  • A good understanding of the discharge process.
  • Possess excellent communication skills to support resolution of medicines management issues.
