We have a fantastic opportunity for a skilled Hospital Discharge Care Coordinator to join our team at Sanctuary Personnel Ltd.

In this role, you will assess patient needs to ensure comprehensive care and coordinate care plans for optimal patient outcomes.

You will need a degree in Social Work or equivalent qualification, current registration with Social Work England, and significant frontline experience as a qualified Social Worker in Adults Social Services within the UK.
