Care Leaver Support Specialist Job Description

We are recruiting a skilled and compassionate individual to join our team as a Leaving Care Person Advisor.

This challenging yet rewarding role requires you to provide expert guidance and support to care leavers, helping them navigate the complexities of transitioning to independent living.

As a key member of our team, you will be responsible for creating, monitoring, and reviewing personalized Pathway Plans that cater to each care leaver's unique needs and circumstances.

Main Tasks and Responsibilities
  • Provide expert advice and support on financial management, housing, education, employment, and community engagement.
  • Develop and maintain strong relationships with local authorities, partner agencies, and community groups to ensure comprehensive support services.
  • Maintain accurate and up-to-date case records, adhering to data protection regulations and ensuring compliance with relevant legislation.
  • Collaborate with colleagues to develop and implement effective support strategies, contributing to the overall success of our services.
What We Offer

We offer a competitive salary of approximately £30,000 per annum, inclusive of holiday pay, and a 36-hour working week with opportunities for career progression and development.
