Sanctuary Personnel Ltd is looking for a highly skilled Care Pathway Coordinator to join our 16+ Leaving Care Service Team in Southwark, working full-time.
Main Responsibilities:- Care Planning: Develop and manage protection plans within a child protection framework, applying a proportionate and ethical approach to exercising authority.
- Team Collaboration: Work with other agencies to promote positive outcomes and ensure an integrated approach, considering the views of professionals and recognizing potential relationship dynamics that may compromise welfare or safety.
- A minimum salary of £45,000 per annum, negotiable based on experience.
- A Management qualification at Level 4 or above.
- Ability to travel independently to required areas.
