Leaving Care Coordinator

Job Description:
This role is focused on providing critical support to care leavers through the Leaving Care Service, which is statutorily mandated to deliver services to individuals aged 18+ and above. The successful candidate will report to Consultant Social Workers or Practice Development Managers and work collaboratively to ensure that Local Authorities maintain regular contact with care leavers, secure suitable accommodation, and facilitate access to education, training, and employment opportunities.
As a key member of the team, you will be responsible for leading cases and working closely with line managers and colleagues to implement the Unit Model. A crucial aspect of this role will be creating Review Pathway Plans that outline the needs assessment and desired outcomes for each care leaver, requiring collaboration with both the individual and relevant professionals.
Essential Qualifications:
- NVQ Level 3 or equivalent child and family training.
- Significant experience working directly with children, young people, and their families.
Approximate Hourly Wage: £24.
