Leaving Care Team Coordinator

Hackney Council's Leaving Care Service requires a highly experienced Personal Advisor to work alongside Social Workers and other team members, reporting to the Team Manager.
This is an ideal opportunity for individuals who possess strong communication skills and the ability to work effectively with young people aged 18-25 living in the community.
The successful candidate will collaborate closely with the Virtual School, accommodation providers, and PMU to guarantee that care leavers receive the necessary support for a seamless transition into adulthood.
A personal Education qualification of NVQ Level 3 or equivalent child and family training is essential, accompanied by an enhanced DBS check.
