Job Summary

An exciting opportunity has arisen for a Band 4 Assistant Practitioner to join the Rhins Home Team. This post could be offered as a development post, with competencies required to be achieved within a timeframe.

This role would offer an opportunity to work as part of the newly developed home team to support patients at home and aid admission prevention and discharge without delay.

  • Can you inspire, enable and empower individuals, Carers and colleagues through compassionate conversations to work collaboratively to achieve what matters within improved health and wellbeing outcomes?
  • Can you demonstrate your skills through re-treatment programs, supporting self-management, support mental well-being and maximising independence?
  • Can you confidently self-coordinate a caseload whilst proactively seeking shared best decisions with individuals and multi-disciplinary multi-agency partners?

Are you aware of Home Teams and how you will be contributing towards working differently in the future to achieve transformational improvements for the health and well-being outcomes for the people of Dumfries and Galloway?

This is a very exciting time with transformational change as we move forward with the development of the pathways within rehabilitation and reablement.

Please note - A current driving licence is an essential requirement of this post. However, if you have a disability which precludes you from holding a driving licence consideration will be given if alternative transport can be arranged.

Due to the ever changing social care climate there is also a requirement to regularly travel to different workplaces. This will be discussed further at interview.

Key Responsibilities

  • Work as part of the Rhins Home Team to support patients at home and aid admission prevention and discharge without delay.
  • Develop and implement re-treatment programs to support self-management and mental well-being.
  • Confidently self-coordinate a caseload and proactively seek shared best decisions with individuals and multi-disciplinary multi-agency partners.

Requirements

  • A current driving licence is an essential requirement of this post.
  • Ability to regularly travel to different workplaces.
