Overview:
We're looking for an experienced Paediatric Nurse to join our team as a Children's Community Healthcare Professional. In this role, you'll be responsible for providing high-quality care to children and families in the community, focusing on complex care packages within the client's own home.

About the Position:
This position requires regular travel across the Dorset region, ensuring seamless care delivery and collaboration with multidisciplinary teams. As a key member of the Acuity Care Group team, you will work closely with the Clinical Nurse Manager and Complex Care Managers to ensure successful patient outcomes. Your primary responsibilities will include:
  • Developing comprehensive care plans and producing clinical documents required for package setup.
  • Liaising with clients and support workers to review running of care packages and make necessary alterations.
  • Collaborating with the CCM to schedule shadow shifts and facilitate efficient sign-off processes.
With a proven ability to adapt and thrive in various clinical settings, including hospitals, community care, and home-based care environments, you'll be well-suited to this role. A valid UK driving license and access to a reliable vehicle are required for this position. We offer a competitive salary, including a car allowance, pension contributions, and 25 days annual leave.
