Alzheimer Scotland is seeking an experienced Post Diagnostic Support Coordinator to join our team in Fife. As a Post Diagnostic Support Link Worker, you will provide critical support to individuals living with dementia and their families, helping them to navigate the complexities of dementia care.

In this challenging yet rewarding role, you will work closely with the Community Mental Health Team at Queen Margaret Hospital in Dunfermline to deliver high-quality support services. Your excellent analytical skills and ability to think creatively will be essential in developing innovative solutions to complex care needs.

The estimated salary for this position is £27,000 - £33,000 per annum, depending on experience. In addition to a competitive salary, you will have access to ongoing training and development opportunities to enhance your skills and knowledge in dementia care.

Key responsibilities include:

  • Developing and implementing tailored support plans to meet the unique needs of each person
