About the Role

We are seeking an experienced and skilled Community Physiotherapist to join our team in South East Staffordshire. As a Community Physiotherapy Specialist, you will be responsible for providing high-quality physiotherapy services to adults within the community.

Key Responsibilities
  • Assess and treat patients with a range of conditions including traumatic brain injury, Stroke, Multiple Sclerosis, Parkinson's, MND, other neurological conditions, spinal cord injury, falls, orthopaedic conditions, amputations, hip and knee replacements, arthritis, fractures, early onset dementia, and multiple pathologies.
  • Develop and implement individualised treatment plans, taking into account patient goals and needs.
  • Work collaboratively with other healthcare professionals to ensure seamless care for patients.
  • Provide training and education to carers, care agencies, and other external partners on an ad hoc basis.
  • Maintain accurate records and reports, adhering to trust policies and procedures.
Requirements
  • Registered Practitioner Status with HCPC.
  • Experience of working as a Community Physiotherapist.
  • Strong communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
  • Flexibility and adaptability in a rapidly changing environment.
