We are seeking a highly skilled Rehabilitation Case Manager with a background in sports therapy or sports rehabilitation to join our team at Health Case Management Limited.

Key Responsibilities:

  • Deliver clinically robust, consistent, and comprehensive telephone assessments to ensure the most clinically appropriate outcomes for our clients.
  • Utilize every contact point with patients as an opportunity to educate and provide clinical support, promoting patient care and well-being.
  • Adept and encourage the highest standards of patient care, including the consistent provision of clinical guidance and accurate, clear advice.
  • Collaborate with our team to provide a seamless and effective rehabilitation experience for our clients.

Requirements:

  • Relevant clinical degree in sports therapy, sports rehabilitation, or physiotherapy.
  • At least 1 year of clinical/hands-on postgraduate experience or previous case management or vocational rehabilitation expertise.
  • Relevant experience in the health, legal profession, or insurance sectors.
  • Strong communication and interpersonal skills, with the ability to work effectively with clients, colleagues, and other stakeholders.
