About the Role

We are seeking a skilled Rehabilitation Case Manager to join our team. As a key member of our clinical services team, you will play a vital role in delivering high-quality, patient-centered care to individuals who have suffered injuries or illnesses.

Clinical Service Delivery
  • Deliver comprehensive telephone assessments to identify the most clinically appropriate outcomes for patients.
  • Assess, plan, implement, and evaluate interventions and clinical pathways to expedite patient recovery and case closure.
  • Utilize the bio-psychosocial model when making contact with patients and making clinical decisions.
  • Provide education and clinical support to patients through every contact point.
Patient Care
  • Share responsibility for patient cases with colleagues, promoting a team working environment.
  • Assist in optimizing service delivery and adopting the highest standards of patient care.
  • Take a forward-thinking, patient-centered approach to overcome case management hurdles.

Essential Skills:

  • Relevant clinical degree (sport therapy, sport rehabilitation, or physiotherapy)
  • 1 year's clinical/hands-on postgraduate experience or previous case management or vocational rehabilitation expertise
  • Current governing registrations of your relevant professional body (e.g., BASRaT or SST, HCPC body)
  • Excellent written and verbal communication skills, good attention to detail, and time management skills

Desirable Skills:

  • Experience with delivery of high customer service and/or proven case management experience
  • Relevant experience (Health, Legal Profession, Insurance) etc. is advantageous
  • CMS UK membership is a bonus
