About the Role

We are seeking a highly skilled Rehabilitation Case Manager to join our team. As a Rehabilitation Case Manager, you will play a critical role in supporting individuals with injuries or illnesses to recover and return to work or their pre-injury condition.

Key Responsibilities
  • Deliver comprehensive telephone assessments to identify the most clinically appropriate outcomes.
  • Assess, plan, implement, and evaluate interventions and clinical pathways to expedite patient recovery and case closure.
  • Utilize the bio-psychosocial model when making contact with patients and with clinical decisions.
  • Provide clinical support and education to patients throughout their recovery journey.
Essential Skills
  • Relevant clinical degree (sport therapy, sport rehabilitation, or physiotherapy).
  • 1 year of clinical/hands-on experience or previous case management or vocational rehabilitation expertise.
  • Current governing registrations of your relevant professional body (e.g., BASRaT or SST, HCPC).
  • Excellent written and verbal communication skills, good attention to detail, and time management skills.
