Senior Care Coordinator Job Description

We are seeking a highly skilled and experienced Senior Care Coordinator to join our team at Sonderwell. As a Senior Care Coordinator, you will play a key role in ensuring the delivery of high-quality care and support services to our clients.

Key Responsibilities:
  • Mobilisation and Resource Allocation: Lead on the management of your caseload of packages, ensuring an expert approach to filling packages and allocating carers to meet client needs.
  • Scheduling and Rota Management: Schedule a consistent and quality-focused rota of carers for your packages, utilizing our internal system to allocate and coordinate carers.
  • Performance Monitoring and Reporting: Utilize our system to report on KPIs such as hours filled, proactively contributing to increasing filled hours of care.
  • Client and Carer Support: Conduct live call monitoring, provide support and guidance to carers, and ensure accurate and timely notes on our system.
Supervision and Monitoring:
  • Phone Observations and Supervisions: Conduct phone observations and supervisions with carers, checking in on well-being and performance.
  • Documentation and Record-Keeping: Ensure accurate and concise notes on our system for yourself and the wider team.
On-Boarding and Interview and Selection:
  • Talent Pipeline Management: Work closely with our Talent team to review pipelines and support the filling of current and future hours of care.
  • Candidate Selection and Placement: Conduct quality competency interviews, assess skills and capabilities, and collaborate with the Talent team to schedule meet and greets for new carers.
  • Induction and Training: Assist with the induction and training of new care workers and team members.
Management:
  • Leadership and Collaboration: Lead on promoting a supportive and collaborative working environment, working closely with management to highlight issues and reduce staffing risks.
  • Team Management: Motivate and guide a small team of care coordinators, ensuring industry and company policies and procedures are followed.

Requirements:

  • Experience: Strong experience gained in a care/health environment, ideally in a senior support or coordination capacity.
  • Skills: Excellent communication skills, flexibility, and pragmatism, with the ability to self-plan and respond to shifting priorities.
  • Qualifications: Proven ability to work at a senior level with the care sector and comply with internal policies and procedures.

  • Communication Style: Confident, clear, and warm communicator with a flexible and constructive approach to carers, clients, and the team.
  • Team Player: Ability to work in a team, remain calm and professional under pressure, and act as a role model for best practice.

Working Environment:

We have a collaborative working environment, with a team that works closely to ensure our clients receive the right care and support. Our office is located in Wokingham, and we have an on-call rota during the week and weekends for emergencies only. We offer a supplement for weekdays and time in lieu for weekend calls, giving you flexibility for those weekday occasions.
