About the Role

We are seeking a highly experienced and skilled Registered Mental Health Nurse to join our team as a Children's Continuing Care Lead. This is an exciting opportunity to work with a dedicated team of professionals who are passionate about delivering high-quality care to children and young people with complex health needs.

Key Responsibilities
  • Lead the implementation of the Children's Continuing Care process, ensuring holistic assessments are undertaken to determine eligibility in accordance with Welsh Government Guidance.
  • Provide clinical leadership and direction to the multi-disciplinary team, ensuring that children and young people receive the best possible care and support.
  • Develop and maintain effective relationships with partner agencies, including social services, education, and health providers.
  • Collaborate with the Children's Continuing Care Lead (Physical Health) and Service Delivery Manager Paediatrics to develop policy changes and implement relevant strategies.
  • Provide training and education to health, social care, and education practitioners on the Children's Continuing Care process.
Requirements
  • Registered Nurse (Mental Health) with current NMC registration.
  • Postgraduate diploma qualification plus Masters' qualification or demonstrable equivalent experience of working with children and young people with complex health needs.
  • Ability to practice within the scope of the NMC Code of Professional Conduct.
  • Evidence of professional development and lifelong learning.
  • Knowledge of health and social care policy, including the Children's Continuing Care Guidance and Social Services and Wellbeing Act.
Desirable Qualifications and Experience
  • Master's Degree level or equivalent relevant experience.
  • Understanding of budgets and budgetary management.
  • Training in evidence-based psychological approaches, such as DBT, Mindfulness-based approaches, EMDR, and CBT.
  • Supervision qualification or experience of project management.
  • WARRN (Child) Safeguarding child and adult as part of mandatory training.
Language Skills

The ability to communicate in Welsh is desirable for this post. If you do not meet the Welsh Language requirements specified, the Health Board offers a variety of learning options and staff support to help you meet these minimal desirable requirements during the course of your employment with us.
