About the Role

This position involves working closely with families to identify their needs, develop support plans, and ensure they receive coordinated support from relevant agencies.

Responsibilities

  • Conduct comprehensive assessments of family needs, strengths, and risks.
  • Develop and implement effective support plans in partnership with families.
  • Work collaboratively with schools, healthcare providers, and other agencies to ensure families receive coordinated support.
  • Maintain accurate records and ensure compliance with statutory guidelines and organizational policies.
  • Advocate for the needs and rights of children and families.
