About the Role

We are seeking a highly skilled and experienced Community Specialist Practitioner to join our team at Manchester University NHS Foundation Trust. As a Community Specialist Practitioner, you will be responsible for providing high-quality nursing care to patients in their own homes and in various community settings.

Key Responsibilities

  • Lead and coordinate nursing teams to provide complex nursing care to adults in their own homes and in community settings.
  • Work closely with multidisciplinary teams, including social workers, GPs, and allied health professionals, to ensure effective patient care.
  • Develop and implement care plans that meet the individual needs of patients.
  • Provide education and support to patients and their families to promote health and well-being.
  • Contribute to the development of the community nursing service, including identifying areas for improvement and implementing changes.

Requirements

  • Registered Nurse with a Community Specialist Practitioner qualification.
  • Experience of working in community nursing, including experience of leading and coordinating nursing teams.
  • Excellent communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
  • Computer literate and able to use electronic records systems.
