About the Role

We are seeking an experienced Community Specialist Practitioner to join our team at Manchester University NHS Foundation Trust. As a key member of our community nursing service, you will play a vital role in delivering high-quality care to patients in their own homes and community settings.

Key Responsibilities
  • Lead and coordinate nursing teams to provide complex nursing care to adults in their own homes and community settings
  • Work closely with multidisciplinary teams, including social workers, GPs, and allied health professionals, to ensure effective patient care
  • Develop and implement clinical skills and knowledge to improve patient outcomes
  • Contribute to the development of our community nursing service, including identifying areas for improvement and implementing changes
Requirements
  • Registered Nurse with a Community Specialist Practitioner qualification
  • Experience of working in community nursing, with a focus on complex care
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals
  • Ability to work autonomously and as part of a team, with a flexible approach to working hours
