About the Role

We are seeking an experienced Community Specialist Practitioner to join our team at Manchester University NHS Foundation Trust. As a key member of our Neighbourhood team, you will be responsible for leading and coordinating nursing teams to provide complex nursing care to adults in their own homes and community settings.

You will work closely with multidisciplinary teams, including social workers, GPs, and allied health professionals, to ensure effective patient care and coordination. Your expertise and experience will enable you to develop new and existing clinical skills, as well as access a range of training opportunities for personal development.

Main Responsibilities
  • Lead and coordinate nursing teams to provide complex nursing care to adults in their own homes and community settings
  • Work closely with multidisciplinary teams to ensure effective patient care and coordination
  • Develop and implement clinical skills and knowledge to improve patient outcomes
  • Collaborate with other healthcare professionals to ensure seamless care delivery
Requirements
  • Registered Nurse qualification
  • Community Specialist Practitioner qualification
  • Evidence of post-qualification specialist study
  • Prescriber V150 or V300 qualification
